Phoenix, AZ (April 20, 2025) – 14-year-old Omar Meraz Hernandez died and two others sustained life-threatening injuries in a crash on 35th Avenue.

A tragic crash in Phoenix, north of Laveen, claimed the life of a teenager and left two adults seriously injured on Saturday afternoon.

At approximately 3:10 p.m., firefighters responded to reports of a collision at the intersection of 35th Avenue and Lower Buckeye Road, where a van had crashed into a pole.

Fire crews rescued two individuals from the wreckage.

Emergency personnel transported a 14-year-old boy, later identified as Omar Meraz Hernandez, to the hospital. 

Despite medical efforts, he succumbed to his injuries. Omar had been riding as a passenger in the vehicle at the time of the crash.

Paramedics also rushed the male driver and a female passenger to the hospital with life-threatening injuries. 

According to Phoenix police, the man had been driving south on 35th Avenue when he lost control of the vehicle, veered off the roadway, and struck a pole. Investigators currently do not believe impairment played a role in the incident.

Authorities closed the intersection for several hours as detectives examined the scene to determine the cause of the crash. 

They reopened the roadway late Saturday night. The investigation remains ongoing.
